Description
Home Health Aide (PCS)-
Community Care Connections is looking for a Personal Care Service provider with the primary responsibility of assisting the individual to be as independent as possible within their own home. They are responsible to follow the individual's Plan of Care as directed by the person's Case Manager and to follow all Medicalodges, Inc Home and Community Based Service policies and procedures.
Community Care Connections is a member of the Medicalodges, Inc. family. Medicalodges offers a continuum of health care options which include in-home services, independent living, assisted living, skilled nursing home care, rehabilitation, specialized care, outpatient therapies, adult day care, as well as services and living assistance to those with developmental disabilities.
Job Responsibilities:
  • Supervise and assist individuals in the daily maintenance and cleaning of their home as outlined in the Plan of Care. This can include household duties such as cleaning, sweeping, mopping, dusting, laundry, changing linens, errands, as outlined in the Plan of Care.
  • Supervise and assist individuals with shopping, meal planning, preparation, and money management as outlined in their Plan of Care.
  • Assist with management of medications and treatments as outlined in the Plan of Care and under the direction of the person's physician, home health agency and/or agency nurse.
  • Supervise and assist individuals with personal hygiene needs such as bathing, grooming, dressing, undressing, toileting and transferring, as outlined in the Plan of Care.
  • Assist individual with home communication (i.e. letters, telephone calls) as appropriate.

  • Written and verbal communication skills
