Join Synergy's dynamic referral operations team as an Electronic Referral Specialist!
We are seeking a detail driven and highly organized professional to serve as the critical first point of contact for all electronically received referrals. In this fast paced and rewarding role, you will ensure that every referral entering our eportals is accurately reviewed, accepted, and routed to the appropriate home health or hospice operation.
About the Role
The Electronic Referral Specialist is responsible for monitoring, managing, and processing all incoming referrals received through our electronic referral portals. This role is essential in ensuring referrals are promptly accepted, accurately triaged, and efficiently directed to the correct agency location.
Key Responsibilities
  • Electronic Referral Monitoring: Continuously monitor designated electronic referral portals (such as CarePort, Mosai/Forcura, hospital ereferral systems, and other integrated platforms) to ensure all incoming referrals are identified in real time.

  • Referral Review & Acceptance: Evaluate each referral to confirm it is appropriate for home health or hospice services, ensuring service type, location, and payer alignment are correct before acceptance.

  • Referral Routing: Accurately route accepted referrals to the correct home health or hospice operation based on service area, payer requirements, and organizational structure.

  • Data Verification: Review submitted referral documents for completeness, verifying that key demographic, clinical, and service details are present before handing off to the intake or eligibility teams.

  • Portal Communication: Respond promptly within the portal to acknowledge receipt, request missing information, or decline inappropriate referrals per established guidelines.

  • Collaboration: Communicate effectively with internal intake, eligibility, and authorization teams to ensure a seamless handoff and prevent workflow delays.

  • Tracking & Accountability: Maintain an organized queue of all referral activity, documenting timestamps, acceptance status, and routing paths to support compliance and reporting.

  • Compliance: Ensure all referral acceptance processes meet regulatory, contractual, and payerspecific requirements.

Qualifications
Required:
  • Minimum of 1 year of healthcare office, patient access, scheduling, intake, or referral management experience; Home Health or Hospice experience strongly preferred.

  • Experience working in or ability to quickly learn electronic referral platforms (CarePort, Forcura/Mosai, hospital EHR referral feeds, etc.).

  • Demonstrated ability to navigate EMR systems; familiarity with Homecare Homebase (HCHB) is a major plus.

  • Basic knowledge of Medicare, Medicaid, and commercial insurance terminology.

  • Understanding of home health and hospice service types, referral sources, and documentation expectations.

Skills and Competencies
  • Exceptional attention to detail and accuracy, especially in highvolume electronic environments.

  • Strong organizational skills with the ability to manage multiple referral queues simultaneously.

  • Excellent written communication skills for professional portal messaging and documentation.

  • Ability to work efficiently in a fast paced, deadline driven setting.

  • Strong critical thinking skills to identify referral discrepancies or misrouted requests.
