Liberty Solutions has a client in need of an Epic Referral Management Assessment Consultant. This resource must have CarePort experience with Epic expertise and a strong operational understanding of referral management, discharge planning and care transitions in order to assess the client's current nursing and social-work referral workflows, compare those workflows and CarePort capabilities against Epic’s native functionality, identify operational and technical gaps, and recommend a future-state approach. The initial contract length is 3 months with the possibility for extension. This is a hybrid role since some travel may be necessary in order to complete the assessment.
Job Requirements:
  • 7+ years of healthcare IT consulting, clinical applications or Epic implementation experience
  • 5+ years supporting Epic clinical, care-management, referral-management or care-transition workflows
  • 3+ years working directly with nursing, social-work, case-management or discharge-planning operations
  • Direct experience with WellSky CarePort, ideally CarePort Transition
  • 2+ years of CarePort experience or at least one enterprise CarePort implementation, optimization or replacement assessment is preferred
  • Completion of at least two formal healthcare technology assessments involving current-state discovery, future-state design, gap analysis and implementation-roadmap development
  • Active Epic certification (Preferred: EpicCare Inpatient, Grand Central, Healthy Planet, Compass Rose) or accreditation relevant to inpatient clinical workflows, care management, referrals or transitions of care
  • Experience evaluating or implementing native Epic functionality as a replacement for a third-party clinical or operational application. 
  • Previous responsibility for producing formal assessment findings, implementation plans and effort estimates.
