Position Overview:
Responsible for overseeing all aspects of the memory care department including care staff
leadership, scheduling, training and supervision, partnering with families and healthcare
providers to ensure residents receive compassionate, person-centered care, maintaining
accurate documentation, medical supply inventory, and promoting workplace and resident
independence, safety and wellbeing in accordance with company Core Values, policies,
physician orders and regulations governing the industry.


Essential Functions:

The essential functions include but are not limited to:


Resident Care and Clinical Oversight
• Must be at work on time/when expected, representing the company in a positive and
professional manner, leading by example in dress, composure and behavior
• Promotes a safe, engaging and therapeutic environment for residents living with various
forms of cognitive impairment helping them to function at their highest level possible
• Conduct pre-admission assessments and ongoing resident evaluations to determine
appropriateness for admission, continued residency, and level of care needs
• Participate in resident move-in and move-out processes collaborating with all departments
• Coordinate physician orders, medication management, ancillary services and outside
healthcare providers to ensure highest quality of resident independence and care
• Ensure timely follow up on changes in condition, incidents, falls, hospitalizations monitoring
residents’ physical, emotional, cognitive, and psychosocial well-being
• Maintain accurate and up to date records, documentation and health-related information
• Continually monitors resident acuity/needs adjusting staffing accordingly
• Serve as primary contact for residents and their families communicating status changes
promptly and professionally participating in care conferences and meetings
• Respond to resident, family, and/or employee concerns with urgency and compassion

Staff Leadership and Supervision
• Recruit, hire, train, supervise, coach and evaluate care/medication staff
• Develop and provide robust orientation and ongoing education to ensure quality care, safety,
infection and abuse prevention, dementia care, proper medication management, etc.
• Ensure staffing schedules meet resident care needs and regulatory compliance requirements
• Conduct documented performance evaluations and corrective actions
• Schedules breaks and manages overtime within budget
Medication Management and Regulatory Compliance
• Oversee all aspects of memory care medication management program, and medication staff
• Order, audit and maintain medications ensuring proper storage and documentation
• Coordinate pharmacy services and timely medication refills
• Monitor/maintain immunization and other required records for residents and employees
• Ensure physician orders are followed, updated and ensures medications are properly stored,
administered and destroyed in compliance with all applicable policies and regulations
• Regularly audit medication staff, records, and carts for compliance, responding appropriately
to medication errors, adverse reactions ensuring physician follow-up
• Maintain readiness for licensing inspections and audits
• Investigate and document incidents, accidents, injuries, complaints and other unusual
occurrences, reporting as required to appropriate agencies timely and accurately
• Maintain all safety and infection control standards
• Assist with policy and procedure development and implementation as needed
• Monitor quality indicators, resident outcomes and service standards
• Participate in emergency preparedness planning, assist with workers compensation, safety
initiatives and risk reduction efforts related to care services and affiliated persons


Other Administrative Duties
• Maintain all health-related equipment in safe, clean and working order
• Operate department within budget
• Attend meetings, prepare reports, complete special projects and other duties, as assigned.

Qualifications:
• Bachelor’s degree in nursing, Healthcare Administration, Gerontology or related field
preferred
• Licensed Vocational Nurse (LVN) or Registered Nurse (RN), preferred
• Valid First Aid and/or current CPR – must have/maintain up to date all required
certification(s), license(s), or other required qualifications for the position
• Ability to pass physical/fit for duty and other skills tests/evaluations for the position
• Minimum two (2) years supervisory experience in a related healthcare setting
• Comfortable working with elderly, dementia and/or disabled persons
• Knows and respects Resident Rights and mandated reporter responsibilities
• Knows and practices safe transferring skills
• Familiarity with electronic health records, Microsoft office applications and other
standard office equipment required for the position
• Must be willing to work any shift(s) to meet the demands of a 24/7 operation

Physical Demands:
• Work is primarily completed in community/senior living environment
• Ability to stand, walk, move about and throughout the entire community, as needed, to
meet resident needs for extended periods throughout the workday.
• Repetitive hand motions for use of computers, typing, phones, and other equipment.
• Ability to lift/transfer up to 35 lbs and use safety equipment (gait belt, transfer board,
Hoyer lifts, etc.) for anything beyond this weight limit, seeking help as needed to avoid
injury and ensure safety.
• Ability to frequently bend, reach, stoop, kneel, push, pull, up to 25 lbs.
• Use of hands, and fingers, to feel, handle, pick up, and manipulate objects as needed.

• Must be able to talk, see and hear with or without prosthetics, for adequate
functioning of these senses.
• Maintain the mental alertness, attention and concentration needed to understand
supervisor instructions, resident/family needs/requests, and respond appropriately.
• Ability to cope with the mental and emotional demands associated with the position.
