Manager of Care Coordination- Bed Placement- Registered Nurse (RN)

Responsibilities

Responsibilities:

  • Collaborates and coordinates with multidisciplinary teams to facilitate a safe and quality experience for patients through the continuum of care.
  • Liaison between all patient care services to ensure appropriate bed is assigned, patient care needs are met and treatment plans are executed in the appropriate level of the continuum
  • Applies advanced clinical judgment and technical proficiency to all patient placement decisions
  • Manage real-time patient volume and eliminate flow delays
  • Assign patient beds based on medical necessity, acuity level, required nursing care hours, and infection control isolation protocols
  • Ensure regulatory compliance with EMTALA, HIPAA, and Joint Commission standards during all patient placement activities

Qualifications:

  • Passion for Care Coordination
  • 1+ years of Case Management experience as a Registered Nurse (RN)
  • 1+ years of nurse leadership experience
  • Education: Bachelors of Science in Nursing (BSN) degree
  • Licensure: New York Registered Nurse (RN) License & Registration
  • Certification: Basic Life Support (BLS)
  • Knowledge of Outlook, Word, Excel
  • Must be flexible and able to work in a stressful environment
Pay RangeUSD $60.28 - USD $72.34 /Hr.Qualifications

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
