Tailored Care Management – Care Manager Extender

Location: Greenville, NC
Compensation: $17–$21 per hour
Employment Type: Full-Time

Position Summary

The Care Manager Extender supports the Tailored Care Management team by assisting Care Managers with member engagement, care coordination, and community resource navigation. Working under the direction and supervision of a Supervisory Care Manager, this role helps expand workforce capacity and strengthen support for members navigating complex care systems.

The Care Manager Extender does not provide clinical services and performs duties within the defined scope of Tailored Care Management functions.

Key ResponsibilitiesMember Engagement & Support
  • Assist with outreach and engagement activities to help members access needed services.

  • Support members in understanding care plans, appointments, and available services.

  • Provide education and information about community resources and Medicaid services.

  • Help identify and address barriers to care, including transportation, scheduling, and communication needs.

Care Coordination Support
  • Document member interactions and activities in the care management system according to organizational policies.

  • Communicate member needs, concerns, and updates to the assigned Care Manager.

  • Assist with follow-up related to referrals, appointments, and service connections.

  • Participate in team meetings and care coordination activities as requested.

Resource Navigation
  • Connect members with community-based services, programs, and support resources.

  • Use community knowledge or lived experience, when applicable, to help members navigate available resources.

  • Assist members with social determinants of health needs, including housing, food, employment, transportation, and other community supports.

Compliance & Collaboration
  • Work under the direction of the Care Manager and within assigned and delegated functions.

  • Maintain member confidentiality and comply with HIPAA and organizational policies.

  • Ensure all activities remain within the approved Care Manager Extender scope.

  • Complete and maintain required training in accordance with applicable state and federal laws and regulations.

Minimum Qualifications
  • Must be at least 18 years of age.

  • High school diploma or GED required.

  • Experience in behavioral health, care coordination, case management, or a related field strongly preferred.

  • Knowledge of Medicaid, social service systems, or community resource navigation is a plus.

  • Strong communication, interpersonal, and organizational skills.

  • Ability to work effectively in a team-based, fast-paced environment.

  • Commitment to equity, inclusion, advocacy, and trauma-informed care.
