ATC Monitor
Full-Time | $19-$22/hour (DOE)At Pathways for Life, we don't just provide services-we ensure they're delivered the right way, every time. We're looking for an ATC Monitor who can balance compliance, relationship-building, and accountability in the field. If you're someone who can spot issues early, communicate confidently with families and caregivers, and follow through until things are resolved then this role is for you.
What You'll Do
  • Conduct in-home and virtual monitoring visits to ensure services match authorized care plans.
  • Catch and resolve compliance risks, missed services, or documentation caps.
  • Maintain audit-ready records aligned with DDD, ALTCS, CMS, and Pathways for Life standards.
  • Build trust with families while ensuring accountability and service integrity.
  • Partner with local office leaders to resolve issues quickly and effectively.

What We're Looking For
  • 2+ years in healthcare, care coordination, onboarding, or social services (DDD/ALTCS preferred)
  • Strong communication skills with the ability to navigate conversations with both caregivers, members, and their families confidently
  • Highly organized with strong follow-up and problem-solving skills.
  • Comfortable working independently in the field and managing multiple calendar invites.
  • Must be comfortable driving to and working in member's homes.
  • Must have a valid AZ Driver's License, clean driving record, reliable transportation, and maintain valid vehicle insurance.
  • Must be bilingual in English and Spanish.

Schedule & Work Environment
  • Monday - Friday, 8:00 AM - 5:00 PM (flexible; occasional evenings/weekends)
  • Fast-paced, detail-driven role with high accountability
  • Local travel required for in-home visits
