Job Description:
The Community Health Center, Inc. cares for 150,000 patients every year at sites across the state. We know that many of CHCI patients have complex lives and taking care of their own health can't always be the number one priority. CHCI is excited to have a Preventative Cancer Screening Navigator join the team to help our patients prioritize these potentially life-saving screenings. The Navigator will work closely with the Population Health team to identify patients who are due for colorectal and/or breast cancer screening. This person will play an important role in the care team by supporting the patient in overcoming any barriers that are preventing them from completing the recommended screenings such as a mammogram or colonoscopy. The Navigator will also help the patient arrange any follow-care that is needed. We know many of our patients have every intention of completing the screenings to which they agreed; but we also know that life sometimes gets in the way! You will be a vital part of helping the patient succeed in completing these screenings.
ROLE AND RESPONSIBILITIES
  • Outreach to patients who are due for preventative cancer screenings
  • Oversee patient mailings
  • Complete a Social Determinants of Health Screening on all patients
  • Address challenges with social determinants of health that are impacting patient's ability to participate in preventative health care
  • Connect with patients directly at our health center sites or in the community to provide personalized education and support regarding preventative cancer screenings
  • Communicate with patient's primary care team, as needed
  • Provider reminders to patients about screening appointments
  • Identify common barriers that are preventing patients from completing preventative cancer screenings
  • Conduct virtual education sessions for patients in English and Spanish
  • Document each patient encounter in the patient's electronic health record.
  • Attend community events and health fairs to educate about the importance of preventative cancer screening
  • Attend CHC site specific meetings, as needed.
  • Other duties as reasonably assigned.

QUALIFICATIONS
  • Associates Degree required.
  • Bilingual- English and Spanish (preferred)
  • Excellent oral and written communication skills.
  • Ability to carry out written and oral instructions.
  • Demonstrated experience in helping patients overcome barriers to care
  • Comfortable with computer data entry, experience working with an electronic health record preferred.
  • Two references
  • Prior experience as an outreach worker preferred

PHYSICAL REQUIREMENTS/WORK ENVIRONMENT
  • This is a hybrid position. Most of the work can be conducted remotely, but the position will require in-person meetings with patients and traveling to various sites.
