Juniper Gardens Center for Nursing & Rehabilitation

We’re looking for a licensed Physical Therapist Assistant (PTA) to provide quality, hands-on care and help our residents achieve their therapy goals in a skilled nursing setting.

Position Details:
  • Status: Full time

  • Shift: Day shift

  • Setting: Long-Term Care / Skilled Nursing Facility

Responsibilities:
  • Follow treatment plans as directed by the Physical Therapist

  • Provide one-on-one and group therapy to residents

  • Track daily notes, treatment progress, and billing

  • Assist with discharge planning and patient education

  • Communicate with therapy team, nursing, and families

  • Participate in care meetings and facility initiatives

Qualifications:
  • Graduate of an accredited PTA program

  • Current state licensure as a PTA

  • Prior LTC/SNF experience is a plus, but not required
