Primary Responsibilities

As a Home Health & Hospice Community Liason, you will develop new referral relationships while strengthening existing partnerships throughout the Phoenix Metropolitan Area.

Responsibilities include:

  • Build relationships with physicians, hospitals, rehabilitation facilities, skilled nursing facilities, assisted living communities, case managers, discharge planners, and healthcare professionals.
  • Promote both Home Health and Hospice services through consistent face-to-face visits.
  • Educate referral partners regarding qualifying diagnoses, eligibility requirements, and available services.
  • Meet with patients and families to explain care options and facilitate smooth admissions.
  • Conduct cold calls, follow-up visits, educational presentations, and community outreach.
  • Represent Billet Health at networking events and community organizations.
  • Maintain accurate marketing documentation and referral tracking.
  • Collaborate closely with clinical leadership to ensure excellent patient experiences.

Qualifications

Required

  • Minimum one year of Home Health and/or Hospice marketing experience.
  • Existing referral relationships throughout the Phoenix Metropolitan Area.
  • Strong understanding of Medicare Home Health and Hospice eligibility.
  • Excellent communication and relationship-building skills.
  • Ability to work independently while managing a designated territory.
  • Valid driver's license, reliable transportation, and auto insurance.

Preferred

  • Experience marketing to physicians, hospitals, SNFs, ALFs, and rehabilitation facilities.
  • Proven success increasing admissions.
  • CRM or EMR marketing experience.
