Job description

We are seeking an experienced and detail-oriented Licensed Vocational Nurse (LVN) Quality Assurance Nurse with strong knowledge of Axxess Home Health software. The LVN QA Nurse will be responsible for reviewing clinical documentation completed by Registered Nurses (RNs) and field clinicians to ensure accuracy, compliance, and quality standards are met before billing submission.

The ideal candidate must have excellent clinical judgment, strong knowledge of home health regulations, and the ability to identify documentation errors that may lead to compliance issues, insurance denials, or delayed payments.

Responsibilities:

  • Review all Registered Nurse (RN) documentation in Axxess for completeness, accuracy, and compliance.
  • Audit Start of Care (SOC), Recertifications, Resumptions of Care, Transfers, Discharges, and Skilled Nursing visits.
  • Verify documentation supports:
    • Patient eligibility and homebound status
    • Medical necessity for skilled services
    • Physician orders and Plan of Care
    • Appropriate visit frequency and interventions
    • Patient goals and measurable outcomes
  • Review OASIS documentation for inconsistencies and notify RN Case Managers of needed corrections.
  • Ensure nursing notes match the patient’s diagnosis, medications, orders, and care plan.
  • Monitor documentation timelines to ensure paperwork is submitted according to agency policy.
  • Communicate with Registered Nurses regarding corrections, missing information, and documentation improvement.
  • Track outstanding documentation and follow up with clinicians.
  • Review orders, medication profiles, wound documentation, and coordination notes.
  • Ensure compliance with CMS Conditions of Participation (CoPs) and agency policies.
  • Assist in preparing charts for audits, ADRs, and insurance reviews.
  • Identify documentation trends and provide education to nursing staff.
  • Maintain confidentiality and follow HIPAA regulations.

Requirements:

  • Current Texas LVN License in good standing.
  • Minimum 1–2 years Home Health experience preferred.
  • Experience using Axxess Home Health software required.
  • Strong understanding of:
    • OASIS documentation
    • CMS home health requirements
    • Skilled nursing documentation standards
    • Plan of Care (485)
    • Physician orders
  • Ability to recognize incomplete or inaccurate clinical documentation.
  • Excellent communication and organizational skills.
  • Strong attention to detail and ability to prioritize multiple tasks.

Preferred Skills:

  • Previous Home Health QA experience.
  • Knowledge of Medicare, Medicaid, and insurance documentation requirements.
  • Ability to educate nurses on proper documentation practices.
  • Experience preventing billing delays and clinical denials.
