The Organization

Operation Samahan (OpSam Health) is a Federally Qualified Health Center (FQHC) that serves the San Diego County community. It was founded in 1973 in a barber shop in Downtown San Diego by Filipino health professionals offering services to immigrants who did not have the finances, linguistic skills, or ability to navigate the health care system. Today, Operation Samahan has evolved into a diverse community health center with services available in National City, Mira Mesa, City Heights, and Rancho Penasquitos. We provide primary care, dental, behavioral health, family planning, integrative health and wellness, healthy eating, civic engagement, and other health-related services to the uninsured, underinsured, and underserved families of all income levels, regardless of their ability to pay.

JOB PURPOSE

As a Nurse Practitioner at a Federally Qualified Health Center (FQHC), you will be an integral part of the healthcare team, working under the guidance of the Medical Director. Your role involves providing comprehensive direct patient care services to individuals of diverse backgrounds and needs. This includes conducting thorough assessments, diagnosing illnesses, developing treatment plans, and prescribing medications when necessary. Additionally, you will play a crucial role in preventive care, health promotion, and patient education. Your dedication to compassionate, evidence-based practice will significantly fulfill Opsam Health's mission of providing accessible, high-quality healthcare to underserved populations.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Perform comprehensive health screenings of patients of all ages.
  • Provides health care services to patients with acute and chronic medical conditions.
  • Order and review diagnostic and laboratory tests and refer patients to specialists as appropriate.
  • Complete progress notes in the electronic health record system in a timely manner.
  • Follow Opsam Health policies.
  • Keep abreast of current evidence-based medical knowledge.
  • Maintains minimum productivity requirements as set by the organization.
  • Maintains satisfactory peer and performance reviews.
  • Participate in quality improvement projects.
  • Performs other duties as assigned.

QUALIFICATIONS

  • Valid Registered Nurse (RN) license and Family Nurse Practitioner (FNP) certification in the state of California.
  • Bilingual in Tagalog/English Strongly Preferred
  • Master's degree in nursing (MSN) from an accredited program with a focus on family practice.
  • Current DEA and BLS certificates are required.
  • Minimum of 1 year of experience as a Family Nurse Practitioner in a primary care or family practice setting.
  • Strong knowledge and proficiency in primary care, including health assessment, diagnosis, treatment, and patient education.
  • Must be COMFORTABLE with all age groups.

Quality Management:

  • Display knowledge of typical signs of human development and ability to assess and provide age-appropriate clinical care.
  • Contribute to the organization's success by participating in quality improvement activities.

Customer Relations:

  • Respond promptly and with caring actions to patient care needs. Acknowledge psychosocial and cultural beliefs.
  • Maintain professional working relationships with all levels of staff, patients and the public.
  • Be part of a team and cooperate to accomplish department goals and objectives.

Safety:

  • Maintain current knowledge of policies and procedures as they relate to safe work practices.
  • Follow all safety procedures and report unsafe conditions.
  • Use appropriate body mechanics to ensure an injury-free environment.
  • Be familiar with the location of the nearest fire extinguisher and emergency exits.
  • Follow all infection control procedures, including blood-borne pathogen protocols.

Privacy Protection & HIPPA Compliance:

  • Maintain privacy of all patients, employees, and volunteer information and access such information only on a need-to-know basis for business purposes.
  • Comply with all regulations regarding corporate integrity and security obligations. Report unethical, fraudulent, or unlawful behavior or activity.
