Job description

The Clinical Transition and Stabilization Services (CTSS) Mental Health Worker will assist emotionally and behaviorally troubled youth with transitioning into and remaining stable within therapeutic foster homes.ย  Within this innovative clinical support program, all services will be provided under the direction of the clinical team, including CTSS Director and team Therapist.

Location:ย Reed Street, Philadelphia
Hourly Rate:ย $21.00/hour
Travel:ย primarily in schools with possibility to homes and community settings. .65 cents for travel reimbursementย 

About the Role

We are seeking a dedicated Behavioral Health Case Manager to join our team and support children and youth in community-based settings. This position requires strong communication skills, the ability to collaborate effectively with multidisciplinary teams, and comfort navigating multiple systems at once.


Responsibilities
  • Provide behavioral interventions and case management to children and youth

  • Collaborate with families, schools, and community partners

  • Maintain clear and professional communication, both orally and in writing

  • Travel throughout Philadelphia to meet clients where they areโ€”homes, schools, and community spaces


Minimum Requirements
  • Bachelorโ€™s degreeย in a mental healthโ€“related discipline (social work, psychology, sociology, counseling, related human services field) from aย regionally accredited program

  • At least 1 year of post-graduate experienceย providing behavioral interventions and case management to children or youth in a mental health or child welfare setting

  • Ability to work effectively as part of a team and manage involvement across multiple systems

  • Strong oral and written communication skills


Preferred Qualifications
  • 1โ€“2 years of experience working with children or youth in behavioral health

  • Experience with mental or behavioral health and case management preferred

  • Comfortable traveling throughout Philadelphia
