Children's Mental Health Case Manager (Mobile Crisis & Family Support)

Location: Reed Street, PA
Pay: $21.71 per hour
Schedule: Day Shift
Mileage Reimbursement: $0.65 per mile

Make a Difference in the Lives of Children and Families

Are you passionate about helping children, supporting families, and creating positive change in your community? Join our Children's Mobile Intervention Services (CMIS) team and play a vital role in helping youth experiencing behavioral health crises remain safe, stable, and successful in their homes, schools, and communities.

As a Children's Mental Health Case Manager, you'll work alongside an Intervention Specialist to provide crisis stabilization, connect families with essential resources, and empower caregivers with the tools they need to support their child's long-term success.

What You'll Do

  • Partner with an Intervention Specialist to provide crisis stabilization services for children and adolescents.
  • Meet with youth and families in homes, schools, and community settings.
  • Complete assessments to better understand each child's strengths, needs, and family dynamics.
  • Maintain a manageable caseload of approximately 15 families.
  • Connect families with behavioral health, educational, social service, and community resources.
  • Coordinate services with schools, child welfare agencies, mental health providers, juvenile justice, and other community partners.
  • Help families implement behavioral strategies developed by the treatment team.
  • Provide coaching, support, and modeling to caregivers to promote successful behavior management.
  • Advocate for children and families while promoting safety, stability, and positive outcomes.
  • Maintain timely and accurate documentation in accordance with program standards.

What We're Looking For

Education

Bachelor's degree from an accredited college or university in one of the following or a related field:

  • Social Work
  • Psychology

  • Counseling
  • Criminal Justice
  • Education
  • Nursing
  • Anthropology
  • Gerontology
  • Political Science
  • History
  • Theology
  • Related Human Services field

Experience

  • Minimum of 2 years of experience in community behavioral health or mental health case management.
  • Experience providing direct behavioral interventions and working with children, adolescents, and families.
  • Knowledge of community resources and behavioral health systems is preferred.
  • Strong communication, organization, and relationship-building skills.
