SUMMARY: The After Care Specialist works as part of the CUA Intervention Team while providing support to the Case Management Team. Intervention encompasses intervention, prevention and support practices for the entire CUA. The CUA Aftercare Specialist will work from objectives established via the DHS Single Case Plan model. This position has overall responsibility for the implementation of the aftercare plan to ensure permanency and that youth are safe.
EDUCATION AND EXPERIENCE:
• Bachelor’s degree in Social Work or related field including sociology, psychology, counseling, criminal justice, or education
• Candidate must be knowledgeable of child and youth development, family dynamics, parting skills, cultural differences and varying learning styles of parenting.
• Valid, Child Abuse, Criminal History and FBI clearances that meet all regulatory requirements
• Good oral and written communication in English.
