Job Description:
SCHEDULE:
  • Full time, benefit eligible
  • 40 hrs/week
  • Flexible scheduling- suggested is Mon- Thrus. 10am-8pm.
  • Preferred licensing - LCSW or MSW
  • Must be comfortable leading classes/groups, conducting one on one sessions, and readiness screenings.

KEY ACCOUNTABILITIES:
  • Develops programs and educates on the components of stress management, relaxation, and coping skills, and the tools and process of behavior change.
  • Provides individual private coaching sessions for participants.
  • Assesses client's needs and provide coaching service with competence in a variety of coaching techniques and/or models that matches the client's stage of readiness to change.
  • Determines potential for negative outcome and adjusts coaching accordingly.
  • Determines and provides relevant community and/or healthcare resources that help support client's solution for change.
  • Helps client work towards forming solutions for health improvement area or problem.
  • Establishes a plan of action and realistic weekly goals with client that support a positive outcome for all clients.
  • Completes and maintains accurate records and reports regarding participant's progress and other required information.

QUALIFICATIONS:
  • Bachelor's degree in social work, behavioral health, health services, or related field
  • Two years of work experience working with mental health and/or substance abuse coaching in a healthcare, therapeutic, physician, and/or other clinical practice setting

PHYSICAL DEMANDS:
  • Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance
  • Job classification is not exposed to blood borne pathogens (blood or bodily fluids) while performing job duties

WORK ENVIRONMENT:
  • Climate controlled office setting with daily movement throughout the facility
  • Interaction with department members and other healthcare providers
