Job Description:
Summary:
The RN - Transfer Coordinator manages the inbound and outbound transfers and direct admits for all Ballad Health facilities and externally within the markets served. This position collects clinical information relative to a transfer request and facilitates the transfer by matching the patient's clinical needs to the facility with the appropriate services and capacity. The Coordinator will collaborate with multiple departments and disciplines within each hospital to foster relationships with referring facilities, physicians, and hospital staff. This individual will work closely with the leadership team to identify, trend, and report barriers to patient throughput.
The Coordinator will perform transfer call center functions in a responsive manner responding to all callers with the aid of multiple computer databases consisting of facility service profiles, electronic medical records, approved clinical protocols, established patient care protocols, and healthcare information. This individual will coordinate and prioritize transfer of emergent, urgent and elective patients to a final destination facility from all potential referral sources to include but not limited to hospitals, urgent care clinics, long term care facilities, and physician offices via a single call to initiate transfer request and expedite decision making relative to a potential transfer. Additionally, the Coordinator will facilitate patient throughput to a final destination facility with an ongoing focus on quality and efficiency. The Coordinator will work to ensure timely transfer, appropriate bed assignment, and accurate patient information.
The Coordinator will ensure accurate and concurrent data collection and maintenance in both electronic medical record and transfer center applications. This individual will triage and prioritize patient information to communicate an accurate clinical presentation of the patient to accepting/consulting physicians. In addition, the Coordinator will complete all training requirements and demonstrates competence in all software applications to include Epic, Soarian, Central Logic, I3 Phone System, etc. The Coordinator will adhere to regulatory requirements such as EMTALA, Ballad Health Ethics and Compliance policies, and quality initiatives. As well as to monitor self-compliance and implements process changes to ensure compliance to regulations and quality initiatives. The Coordinator will communicate effectively with physicians and referring facilities regarding all patient transfers & potential bed placements. This individual will use problem-solving skills to determine the course of action to generate the best patient clinical outcome, with regards to physician preferences, and the referring/accepting hospitals. Critical thinking skills are utilized to insure every patient transfer promotes the well-being and safety of the patient. Additionally, the Coordinator will adhere to established policy and procedure and standards of care; escalates issues through the established Chain of Command in a timely manner. The Coordinator will function as a liaison between physicians & hospital staff.
Requirements:
Associate's degree from an accredited Nursing program required, Bachelor's degree preferred. Thre years clinical nursing experience required. Must be proficient in keyboarding, minimum typing of 45 words per minute, and demonstrate exceptional computer skills.
Licenses and Certifications:
Valid and active RN licensure in appropriate stateCPR required
BLS is required within 6 months of hire.
