Job description

BACS is looking for Clinical Care Coordinators for our brand new Community Wellness and Response Team (CWRT). CWRT is formerly known as Alternatives to 911. This innovative program works collaboratively with 988 and Sacramento County to provide urgent response for community members experiencing a mental health crisis to reduce the need for emergency services and provide community based resources.
Our CWRT operates 24/7, 365; All staff rotate assignments for dispatch and response. Responders go out as a team of 2 in company vehicles to providing in-field response to de-escalate and resource community members. BACS CWRT staff are working together with 911/988 dispatchers, law enforcement, and our own dispatchers.
No two days are the same at the CWRT, we are helping prevent people from getting into the system, and helping them access resources to prevent the need for law enforcement and crisis services.
Schedule: All NOC shift positions 12:00 am - 8:30 am with schedule of choice
(Sunday-Thursday), or (Friday - Tues)
DUTIES AND RESPONSIBILITIES:
  • Disptaching and response for crises referred by 911/988 for alternative mobile crisis response.
  • Work closely between mobile teams, dispatch and county law enforcement, 911 or 988 to deescalate crisis situations with one or more individuals
  • Provide individual, family, group counseling, individual rehabilitation, and case management/brokerage for individuals in the field.
  • Utilize Evidence-Based Practice for the designated population/program to assure high-quality results and intended impact.
  • Assess an individual's risk status including risk-based symptoms, substance use, environment, physical conditions, etc, and intervene appropriately.
  • Creating a service plan that aims at an individual getting better/healthy and accomplishing service goals that are person-centered and belong to the client.
  • Outreach and progressive engagement and reporting disengagement to the team immediately.
  • Assessment, screening referrals, service planning, community planning, and benefits coordination support wellness and recovery principles.
  • Documents and maintains all data collection, reporting, and charting records in accordance with BACS and other relevant county regulatory policies/agencies.
  • Participates in supervision, staff meetings, consultation, travel, crisis/emergency coverage, weekend coverage, as well as, agency-wide meetings and training, as required by the manager.
  • Other duties as assigned.

QUALIFICATIONS:
  • Master's Social Work (MSW), Masters Marriage Family Therapy (MFT) or Masters Practitioner in Clinical Counseling (MPCC) required
  • Registered with the BBS as an Associate ASW/ Licensed LCSW or Associate MFT/ Licensed LMFT
  • 2 years of social service experience required
  • Bilingual in a threshold language preferred.
  • Valid California driver's license, personal vehicle, and proof of personal vehicle insurance coverage are required. DMV printout required.
  • Extensive local travel is required, vehicle reimbursement is provided. Clinical Care Coordinators transport clients in their personal vehicle.

  • Highly competitive compensation. $40/hourly, non-licensed but BBS registered AMFT, ACSW/APCC and $104,000 annually (exempt) for licensed BBS registered LCSW/LMFT/LPCC.
