Overview:
Join a compassionate team dedicated to delivering high-quality care where it matters most — in patients’ homes. As a Home Health RN, you will provide personalized nursing care, helping patients recover from illness, manage chronic conditions, and maintain independence in the comfort of their own environment.
What you will be doing:

  • Provide skilled nursing care to patients in their homes, including private residences, assisted living, and adult foster homes

  • Perform comprehensive patient assessments and develop individualized care plans

  • Monitor patient progress and adjust care plans to support optimal outcomes

  • Educate patients and families on disease management, medications, and recovery

  • Collaborate with physicians and interdisciplinary teams to coordinate care

  • Maintain accurate, timely documentation of assessments, treatments, and outcomes

  • Advocate for patients and support their safety, comfort, and well-being


What you must have:

  • Current RN license in Oregon

  • BLS certification

  • Minimum 1 year RN experience in acute care

  • Valid Oregon driver’s license, reliable vehicle, and current auto insurance


 Nice to have:

  • Home Health, Community Health, or Critical Care experience

  • BSN


Other Information:
Schedule

  • Monday – Friday, 8:00 AM – 4:30 PM

  • Rotating weekend coverage shared among clinicians

  • Maximum 40 hours per week
