Registered Nurse (RN) โ Oregon
Care at Home Oregon is seeking a compassionate and experienced Registered Nurse (RN) to join our growing home care team. This position is ideal for an RN who is passionate about providing quality care in the home setting and has required experience working with individuals with Intellectual and Developmental Disabilities (IDD).
As part of our team, you will conduct client assessments, develop care plans, train caregivers, and complete supervisory visits to ensure our clients receive the highest quality of care.
Responsibilities
Complete initial nursing assessments for new clients.
Develop and update individualized care plans based on each client's needs.
Conduct required supervisory visits every 90 days.
Train new caregivers, including medication-related training.
Monitor the quality of care being provided in the home.
Collaborate with clients, families, caregivers, and the Care at Home team.
Maintain accurate and timely documentation.
Qualifications
Current Oregon Registered Nurse (RN) license.
Experience working with individuals with Intellectual and Developmental Disabilities (IDD) is required.
Previous home health or community-based nursing experience is preferred.
Strong assessment, communication, and organizational skills.
Experience using Electronic Medical Records (EMR) is preferred.
Ability to work independently and manage multiple responsibilities.
Reliable transportation and the ability to travel to client homes.
Pay
$50.00โ$60.00 per hour
