Connected Home Health is seeking experienced & caring Home Health Registered Nurse (RN) Case Managers to join our awesome team! Do you desire a flexible work schedule? Are you a Registered Nurse that is committed to providing LIFE CHANGING SERVICE to patients in the patient's home? Our RN Case Managers are passionate about helping patients pursue their road to recovery. Connected serves patients in the greater Portland, OR area with territories available on the Southeast or Southwest side. The ideal candidate is a dedicated patient advocate who is home health savvy, compassionate, experienced working with an interdisciplinary team, flexible, driven, creative, technologically adept, fun (!), quality focused, positive, and overall awesome. Our Registered Nurses are supported by trained LPNs.
Salary: commensurate with experience $50-55
Essential Job Functions/Responsibilities
  • Provides direct patient care as defined in Oregon's Nurse Practice Act
  • Implements plan of care
  • Provides accurate and timely documentation consistent with the plan of care
  • Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care
  • Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines to the registered nurse

Position Qualifications
  • Graduate of an accredited practical nurse or vocational nursing program
  • One year nursing experience preferred
  • Currently licensed as a licensed registered nurse in the state
  • Complies with accepted professional standards and practice
  • Demonstrates good verbal and written communication, and organization skills
  • Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order
  • Possesses and maintains current CPR/BLS certification from AHA
