We are seeking a full-time Registered Nurse for the Hospital at Home (HaH) Program at Fargo Sanford. In this innovative role, an RN would have the opportunity to care for a wide range of inpatient medical-surgical patients in the patient's home setting and act as a command center nurse, virtually connecting with the patient from our hub located at SMCF. What you will do:
Utilize Medical Surgical nursing skills to care for a select group of inpatient status patients in the patient's home setting utilizing remote technology and innovative tools to accomplish your nursing care.
This is a dual role position with opportunity to work as the hospital at home nurse and the command center nurse
Hospital at Home Nurse Responsibilities:
-12 hr night shifts with some weekend and holiday requirements
-Visit patients in their home setting and complete nursing assessments, education, medication administration, lab draws, admission/discharges, etc. on up to 5 patients during day time hours
-1 in home visit per patient per day (minimum)
Sanford car will be provided
-Command Center Nurse:
-12 hr night shifts, rotating weekends and holidays
-Responsible for triaging patient calls
-Activate HaH nurse to the patient home
-Coordinate remote visits for the interdisciplinary team
-Work in our remote hub located at SMCF
What you will need:
-ASN or BSN Degree in Nursing
-Active Registered Nurse License in North Dakota & Minnesota
-Active Driver's License

Summary

The Registered Nurse (RN) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care to patients.

Job Description

Collaborates with other inter-professional colleagues, including physicians, to plan, implement and evaluate care. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned clinical setting. Cares for patients in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up as patients move along the continuum of care. Responsible for the planning and coordination of care, patient assessment, patient education, triage, and various other nursing interventions. Functions within the scope and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.

Qualifications

Graduate from an accredited nursing program required, including, but not limited to, American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Bachelor's Degree in nursing preferred.
Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications.
