8 am - 4:30 pm | M - F | 32-40 hr/wk | On-call Rotation
RESPONSIBLE TO
Director of Missouri Slope at Home
PRIMARY JOB FUNCTION
  • Assess condition of Missouri Slope at Home Clients, coordinate care plans and communicate with Missouri Slope at Home RN Manager, Director, Service Manager, clients, client families, and and ND Home and Community Based Services Personnel.
  • Ensure training requirements are satisfied for all employees through ND Qualified Service Provider program, including specific nursing tasks on ongoing nurse oversite of nursing tasks.
  • Meet with potential clients to provide information about At Home Services
  • Participate in (QAPI) Quality Assurance & Performance Improvement program and participate in quarterly QAPI meetings as needed.
  • Assist with coordination of Infection Prevention and Control program.
  • Participate in On-call rotation

ESSENTIAL JOB FUNCTIONS
  • Demonstrates responsible, flexible & dependable behavior in performing duties.
  • Must have the ability to read, write, understand and speak the English language to communicate effectively with residents, family members, and co- workers and to complete required documentation in the, care records and established forms etc.
  • Must be knowledgeable of standard nursing process and practice, as well as regulations and guidelines governing home care programs. Complete thorough documentation on permanent client records, as well as for various required records pertaining to OSHA, Risk Management, training records and policies.
  • Must be able and willing to assess and develop care plans for all residents regardless of gender, religion, color or creed.
  • Demonstrates support of and compliance with; clients rights in all regards including confidentiality of resident information and holds staff accountable to those rights as well.
  • Assist with Coordination of orientation programs for all staff.
  • Coordinate and conduct PCA/QSP/CNA skills and training to all levels of employees.
  • Assist with monitoring Infection Control policies, procedures, and practices.
  • Monitor, analyze, intervene, and report on infection control issues for clients and staff.
  • Assist RN Manager with information for Quality Assurance and Performance Improvement (QAPI) Committee as needed.
  • Provide information and direction to Missouri Slope at Home staff to facilitate and ensure compliance with infection control and safety issues.
  • Review and evaluate client incidents and coordinate with the RN Manager any procedure changes necessary to promote client safety.
  • Attend workshops and meetings related to Missouri Slope at Home programing, staff development, infection control and safety, and any others deemed relevant to the RN position.
  • Attend continuing education trainings as needed to maintain credentials.

NURSING CARE RESPONSIBILITIES:
Assist the RN Manager with all aspects of nursing care for Missouri Slope at Home clients and training staff competencies relating to Home Care when appropriate. Assist the RN Manager with all aspects of nursing tasks as assigned for clients I their residential homes.
  • Assessment and assistance with client admissions.
  • Assist RN Manager with development, review and maintenance of care plans utilizing the approved electronic record system.
  • Communication with RN Manager, clients and/or family members in a timely manner (e.g. any concerns, changes in condition, care routine, etc.).

FUNCTIONS
  • Provide leadership training/oversight and /or serve on committees, taskforces, etc. as assigned.
  • Must be able to stand, walk, talk, and push, pull or lift up to 25lbs up to 50 lbs. as needed. will frequently be required to sit at a computer, stand and use all motor function to tend to client needs. This includes the ability to reach, stoop, kneel, bend, squat, crouch, or crawl, and speak the English language. Is able to complete all tasks of position as outlined. Must be comfortable driving to and from client homes in personal vehicle.
  • Will execute duties in both home settings (of contracted client) and office setting at Missouri Slope. Able to travel to client's homes, and sometimes accompany/drive members outside the home.
  • Understands proper donning and doffing of protective or safety attire, such as gloves and mask. There will be exposure to hazardous materials including blood borne pathogens or household chemicals which requires knowledge of basic chemical safety/infection prevention. Work could involve exposure to plants, animals, dust, and other allergens within the environment.

MINIMUM QUALIFICATIONS
  • Registered Nurse with a current, unencumbered North Dakota license. Previous experience educating
  • and training preferred. Valid Driver's License required. Previous experience working with older adults
  • Successfully pass a drug test, background check, including criminal history, certification registries, and employment history
  • Completion of training as required by Missouri Slope
  • Employees at Missouri Slope are required to receive the annual influenza vaccination as a condition of employment. Successful candidates must either get the vaccination (which is covered by the employer), provide proof of a current vaccination, or obtain an approved accommodation on their start date.
