Physical Therapist (PT) – Home Health (PRN / Hourly)

Position Overview

Signature Healthcare at Home is seeking a compassionate and skilled Physical Therapist (PT) to join our Home Health team on a PRN/hourly basis in beautiful Lincoln City, OR. This role offers flexibility and the opportunity to make a meaningful impact by providing patient-centered care in the comfort of patients’ homes along the scenic Oregon Coast.

The ideal candidate is independent, patient-focused, and passionate about delivering high-quality rehabilitative care to diverse populations.

Key Responsibilities
  • Conduct comprehensive physical therapy evaluations in the home setting
  • Develop and implement individualized plans of care based on patient needs and physician orders
  • Provide hands-on therapy to improve mobility, strength, balance, and functional independence
  • Educate patients and caregivers on treatment plans, exercises, and safety strategies
  • Accurately document visits, progress, and outcomes in the EMR system
  • Collaborate with interdisciplinary team members including nurses, OT, SLP, and physicians
  • Monitor patient progress and adjust treatment plans as needed
  • Ensure compliance with all regulatory, ethical, and company standards
Qualifications
  • Graduate of an accredited Physical Therapy program
  • Current Oregon Physical Therapist license (or ability to obtain)
  • Minimum of 1 year of experience preferred (home health experience a plus)
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal skills
  • Reliable transportation, valid driver’s license, and auto insurance
  • Ability to work independently and manage a flexible PRN schedule
Schedule & Compensation
  • PRN / Per Diem / Hourly – flexible scheduling based on availability and patient needs
