Description

American Traveler is seeking an experienced RN for a Home Health Visit Nurse position in the Port Angeles and Sequim areas, requiring a valid WA RN license and home health or acute care experience.

Details
  • Home health care setting serving patients in their place of residence in the Port Angeles and Sequim areas
  • Post-acute care patient population with a variety of nursing needs as delegated by a Case Manager RN
  • Full-time schedule of 5x8-hour day shifts, 40 hours per week
  • Weekend and holiday coverage required
  • 13-week contract
  • Charting performed in Epic (experience required)
Requirements
  • Active WA State RN license or compact license accepted in WA currently in hand
  • Current BLS certification
  • Associate's degree required; BSN preferred
  • Minimum of 1 year of recent experience in home health, acute, or long-term care
  • Valid WA driver's license with a driving record in compliance with facility standards
  • Proof of automobile insurance and reliable personal vehicle for traveling to patient residences
  • Cell phone with activated voicemail required
  • Home internet connectivity required (work laptop provided)
Additional Information
  • Provides nursing visits and continuity of care for a caseload of patients under the overall planning and coordination of the Case Manager RN
  • Delivers nursing care in patients' places of residence in accordance with physician orders and applicable regulations
  • Keeps the Case Manager RN informed of patient progress, recommended actions, physician orders, and plans for upcoming visits
  • Expected to apply full working knowledge of nursing principles and demonstrate independent problem-solving in complex and diverse situations
