Position: Logansport Transition Specialist 

Location:  Logansport Juvenile Correctional Facility    1118 South State Road 25, Logansport, IN 46947

Duration: 8 + Months (Possibility of Extension)


The Transition Specialist for Indiana's Department of Correction: Division of Youth Services provides oversight to all transition-related practices in the State's juvenile correctional facilities.

Job Responsibilities may include, but are not limited to: 

  • Facilitate the implementation of a life skills curriculum, titled Next Steps.  
  • Utilize the Transition Coordinator Handbook in an effort to standardize the transition practices.  
  • Evaluate the five "Transition Consultations" to be completed by every student under the guidance of an assigned teacher.  
  • Participate every week in Administrative Release Committees (ARC) in each facility to confirm and reinforce the student's reentry plans.  
  • Do the follow-up phone calls to released youth at 1, 3, 6, 9, and 12 months.  
  • The Transition Coordinators will represent the Indiana Department of Correction- Division of Youth Services at many conferences and meetings.  
  • The Transition Coordinators will assist in the development of a partnership with Indiana's Department of Workforce Development, HIRE, and Indiana University’s HOPE Mentoring.   
  • The Transition Coordinators will assist in the development of the individualized Education Learning Plan for each youth upon entry at the facility.  
  • Work with state, county and private agencies to provide re-entry services for youth transitioning back into the community. 
  • Assist students with obtaining social security cards, state identification, driver’s licenses, birth certificates, and additional documentation. 
  • Assist students with transitioning back to public schools
  • Assist students with employment, apprenticeship programs, and military service information
