Job Description

Position: OT - Home Health

Position Summary
Provides occupational therapy services in clients' homes as prescribed by the physician and offers consultation and education to agency staff.

Level of Responsibility
Works under agency policies and client care plans. Reports to the Argus Home Care administrator or designee and supervises assigned personnel.

Responsibilities

  • Patient Care:

    • Conduct initial and ongoing evaluations, including OASIS assessments.
    • Develop and update care plans with the physician and care team.
    • Identify barriers, set goals, provide treatment, education, and equipment.
    • Coordinate services and plan discharge.
    • Evaluate care effectiveness.
  • Documentation:

    • Record evaluations, treatments, and client responses promptly.
    • Document conference outcomes and quality assurance data.
  • Attend care conferences, staff meetings, and in-service programs.

  • Provide education and consultation to clients, families, and staff.

  • Supervise aides, therapy assistants, and paraprofessionals.

  • Maintain professional knowledge through education, professional organizations and workshops.

  • Perform other related duties as needed.

Qualifications

Position Qualifications

  • Active Colorado OT license
  • Graduate of an occupational therapy curriculum approved by the Council of Medical Education of the American Medical Association, and the American Occupational Therapy Association
  • Experience in Home Health preferred, but not required
  • Valid Driver's License & clean Motor Vehicle Record (MVR)
