Job description

Locations: Viking (Little Canada), Furness (Maplewood), Howard (Maplewood) and Caswell (White Bear Lake).
POSITION PURPOSE:
The Health Services Appointment coordinator is responsible to schedule physician ordered clinic appointments as well as to accompany residents on appointments. This position requires organizational skills and systems management for coordinating the residents' schedules with the clinic(s) and relaying the Health Services staff information accurately and arranging transportation for all the appointments. Familiarity with developmental disabilities along with the ability to advocate for the residents in an independent setting is needed. Flexibility within the part-time hours is sometimes necessary. This is a non-exempt position.
NATURE AND SCOPE:
The Appointment Coordinator is supervised by the Director of Nursing who is supervised by the Vice President of Health Services who reports to the President. The position requires accurate communication and documentation, which promotes advocacy for the residents. Exposure to bloodborne pathogens in this position is in OSHA category II, with no regular exposer to blood or body fluids. The position identifies and responds to resident emotional and social needs; however, intervention in this area does not fall within the meaning of psychotherapy.
JOB RESPONSIBILITIES:
  • RESIDENT CARE AND TREATMENT Handles and administers medications and treatments; assists with transfers and direct care duties as needed; schedules appointments and transportation; accompanies residents on appointments; communicates appointments information to residents/staff in a timely manner; responds to residents' physical needs and informs Phoenix Health Services staff or clinic personnel; assists with vital signs; assists to assure new eye glasses, hearing aids or equipment ordered as consults call for; documents clinic appointments information and notifies ID team members nurses of information as needed; offers quality assurance information regarding clinic/professional vendors and assists in seeking service as vendors requested.
  • REGUATIONS COMPLIANCE Adheres to all policies, procedures and philosophy of The Phoenix Residence, Inc. and all rules and regulations of licensing agencies; meets all mandatory hours of training required; documents all canceled appointments in the medical record. Maintains resident medical records with established policies and procedures.
  • HUMAN RELATIONS Communicates effectively with residents, their families, staff, health professionals and other community members; maintains and fosters professional relationships through use of good verbal and written communications; advocates for residents through telephone correspondence and expediency of clinic appointments, while at clinic, in team meetings, nursing report, through documentation on report boards and in medical record and through development of staff/residential in services to foster optimal health care reflected upon clinic visits. Notifies peers and DAC or upcoming appointments. Demonstrates flexibility in scheduling of appointments to accommodate residents' schedules.
  • DEVELOPMENT OF POSITION Assists in training and preventative in servicing of staff in areas of need reflected upon in clinic appointments. Shares quality assurance concepts when applicable; attends mandatory and other job related in services; educates appropriate staff to new procedures/recommendations by consultants. Assists other Phoenix sites in establishing systems for management of appointments and transportation.
  • SAFETY Complies with safety, lifting and infection control procedures; follows med passing standards of practice. Responds to hazardous situations and reports potentially dangerous hazards promptly.
  • ASSUMPTION OF ADDITIONAL RESPONSIBILITIES, AND REQUIRED OR REQUESTED.

QUALIFICATIONS
  • A High School Diploma or its equivalent.
  • Current certificate to pass medications or the ability to obtain within six months of employment.
  • Experience working with residents with developmental disabilities preferred.
  • Ability to lift or transfer, occasionally lifting up to 75 lbs.
  • Ability to sit, stand, bend, reach, push/pull wheelchairs.
  • Ability to read, write, and speak the English language, with composition skills for communications.
  • Ability to drive between sites and transport residents to and from medical appointments; holds valid driver's license and maintains a safe driving record.
  • Ability to hear and see adequately to communicate about appointments.
  • Familiarization with computers or ability to participate in training for Health Services related projects.
