What You'll Do:

As an LPN with Genesis Home Care, you'll provide skilled nursing care in patients' homes under the direction of the attending physician and case manager. You'll play a key role in improving patient outcomes, preventing re-hospitalizations, and helping patients regain independence safely at home.

Key Responsibilities:

  • Deliver skilled nursing care according to physician orders and care plans

  • Monitor patient conditions and communicate changes appropriately

  • Administer medications and treatments as prescribed

  • Educate patients and families on disease processes and preventative care

  • Complete accurate and timely clinical documentation

  • Follow agency policies, procedures, and Medicare/Federal guidelines

  • Participate in trainings and team meetings
    Requirements:
  • Current Licensed Practical Nurse (LPN) license in the state of employment

  • Minimum of one year of LPN experience in an acute care setting

  • Home health experience preferred

  • Valid driver's license and active auto insurance

  • Ability to meet physical job requirements
