Job description

Come home to Aloha. Providing personalized skilled nursing, intermediate, long term, respite and hospice care in a warm, plantation-style atmosphere.
Summary of Position:
Works closely with the nursing management team in accomplishing the mission of Hale Ho Aloha and providing optimal care to its residents. The position entails application of the nursing process through assessment, nursing diagnosis, planning, implementing, documenting, and evaluating plans of care for assigned residents. Performs resident care in accordance with established policies and procedures, and standards of nursing practice as delineated by Hale Ho Aloha, including rehabilitation and restorative nursing and therapeutic services.
Essential Duties:
  • Collaboration with the Director of Nursing, Charge Nurse, the physicians and the interdisciplinary team in enforcing the policies, procedures, and regulatory requirements of Hale Ho Aloha.

  • Maintains open communication with the nursing management staff and other long-term associates and disciplines.

  • Integrates established care plan interventions while performing resident care duties and provide supportive services.

  • Participates in the assessment of residents' functional status and emotional, social, and medical therapeutic needs.

  • Supports and promotes resident rights, dignity, and quality of life.

Requirements:
  • Current State of Hawaii Registered Nurse License
  • Basic Cardiac Life Support (BCLS), Level C required. Re-training once every two years.
  • First Aid & CPR Certification
  • Physical exam (within 1 year)
  • 2 Step TB, Followed by 1 Step TB (within 1 year)
  • 2 References Required
  • Experience in hospice, long term or acute care preferred
  • Flexibility to work other times as needed and to attend mandatory training.
  • Excellent written, verbal, non-verbal, and active listening communication skills.
  • Experience with PointClickCare preferred.
