Our dedicated team of nurses, aides, social services, spiritual care professionals, and volunteers work together to provide comfort, solutions, and peace to those in need. Job Overview:

The RN Hospice plans, organizes, and directs hospice care, with a focus on community health education and experience. As an RN Hospice, you will utilize your nursing skills to provide professional care, assess patient needs, develop care plans, administer medications, and coordinate patient care. You will also have the opportunity to work closely with the interdisciplinary group to ensure the emotional well-being of patients and their families.

Duties & Responsibilities:

Patient Care

  • Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
  • Provide professional nursing care using the nursing process
  • Develop care plans and establish goals based on nursing diagnosis
  • Administer medications and treatments as prescribed
  • Assist patients with activities of daily living and self-care
  • Act as a Case Manager to coordinate patient care

Communication

  • Maintain accurate clinical notes and records
  • Communicate with physicians and community health professionals
  • Teach patients and families self-care techniques
  • Ensure a safe environment for patients
  • Collaborate with the interdisciplinary group to meet emotional needs

Additional Duties

  • Participate in on-call duties
  • Coordinate necessary items and services
  • Supervise ancillary personnel
  • Maintain professional knowledge through continuing education
  • Participate in quality assessment performance improvement activities

Job Requirements:

  • Graduate of a National League for Nursing accredited school of nursing
  • Minimum of two years of nursing experience, preferably in public health, home care, or hospice
  • Current licensure to practice professional nursing
  • Excellent observation, problem-solving, and communication skills
  • Ability to organize and prioritize workload independently
  • Current CPR certification
  • Valid driver's license and reliable transportation
