Brockton Home Health Care AFC is looking for A registeredNurse (RN)/Case Manager to manage and support our Adult Family/Foster to makesure the our programs runs smoothly.

Jobdescription

       Responsible for overall supervision of all HomeVisits RN's;

       Completing the MDS-HC and other clinicalassessments;

       Developing and reviewing on an ongoing basiseach member's AFC/GAFC plan of care;

       Training, evaluating, and supervising AFC/GAFCcaregivers in conjunction with Program Director;

       Reviewing the physician summary;

       Monitoring the health status of all members toensure that all needed AFC/GAFC is properly delivered;

       Reporting changes in the health status of anymember to the member's physician;

       Periodically reviewing AFC/GAFC caregiver logs;

       Conducting admission on-site visits for newmember;

       Completing a nursing progress notecorresponding with each on-site visit or encounter;

       Submitting a semiannual health-status report tothe member's primary care physician;

       Planning for an implementing discharges fromthe AFC/GAFC program;

       Conducting an orientation for each AFC/GAFCcaregiver before the AFC caregiver begins personal care;

       Providing ongoing training to AFC/GAFCcaregivers on health and aging

       Any other office duties that may be assigned byProgram Directors.

Qualification

          Demonstratedability to coach, train and provide staff development

          Proficiencyin Microsoft (Word, Excel and PowerPoint)

          MS or Bachelor'sdegree in Nursing

          Must befully licensed RN by Mass Board of Registration

          At leastfive years of previous home health care experience
