Schedule: 8:00 AM to 4:30 PM, Monday through Friday

Momentum is hiring an Program Coordinator to support disabled individuals in an Adult Day Program (ADP) setting. As a Program Coordinator, you will be responsible for promoting the programs objectives, by creating an environment where people feel welcomed, invited, and supported in their personal and group exploration of the projects and hands-on activities. Our Assistant Program Managers have the unique opportunity to build a rewarding career in healthcare and to develop caring relationships.

Program Coordinator Essential Duties:
  • Respects and promotes the right of individuals
  • Collaborates with Program Manager to promote program objectives
  • Assists the Program Manager in the supervision of program staff
  • Understands and manages regulations for Community Care licensing
  • Maintains a clean, orderly, and safe environment
  • Inventories and orders program supplies
  • Conducts service observations daily and accurately document observations
  • Effectively coaches instructors and direct support staff to improve people supported program implementation
  • Prepares accurate annual and semi-annual individual service plan reports (ISP's)
  • Monitors quality assurance and individual satisfaction with service provision
  • Works directly with people supported, families, and service providers regarding ISP's
  • Assist people attending the program with program activities and community-based activities
  • Assist people attending the program with communication, feeding, mobility, toileting, hygiene, and other activities of daily living
  • Follows and implements company policies
  • Performs other duties as assigned to meet departments and/or business needs

Program Coordinator Qualifications:
  • High school diploma or equivalent; required
  • College courses or credits in human services; preferred
  • Minimum of one (1) year of supervisory experience; required
  • Minimum of five (5) year of experience working with adults developmental disabilities; required
  • Ability to maintain confidentiality
  • Communicate and work professionally and cooperatively with others
  • Successfully pass all post offer, pre-employment screening requirements, including criminal background check, drug screening, physical exam, and provide proof of eligibility to work in the United States
  • Ability to run, push, pull, bend, squat, kneel, and lift up to fifty (50) pounds
  • Valid and current proof of legal right to drive in California; preferred
