Job Title: CLINICAL NURSE NAVIGATOR I

Job Summary: The Clinical Nurse Navigator I is a registered nurse responsible for the seamless coordination of care across the continuum for patients with complex, integration-sensitive diagnoses. Acting as a patient advocate, the Navigator coordinates care in conjunction with physicians from diagnosis or admission through discharge or end of life. Responsibilities include educating patients and families on the healthcare experience, implementing, coordinating, monitoring, and evaluating care options, services, and resources. The Navigator links patients and families to appropriate resources, proactively enhances the patient experience clinically and in service, and provides an exceptional patient experience. Additionally, the Nurse Navigator assists in quality improvement initiatives by gathering, tracking, and researching quality metrics, monitoring outcomes such as patient satisfaction, compliance with treatment regimens, and loyalty. The role involves interaction with department nurses, orientation and training of incoming clinical nurses and patient care assistants, and performing other duties as assigned.
Required Qualifications:

  • Five (5) years of nursing experience and three (3) years in the area of specialty.
  • Neurosciences: Mandatory stroke education requirements per certification agency.
  • Oncology: Minimum of one (1) year of oncology nursing experience.


Preferred Qualifications:

  • Acute Rehab: Prior Navigator experience.
  • Care Management: Prior Sepsis RN experience preferred. Prior experience as a Case Manager, Navigator, or Health Educator.
  • Hoag Orthopedic Institute (HOI): Prior Orthopedic Navigator experience.
  • Community Health: Bilingual skills, especially Spanish.
  • Prior research experience.


Required Education:

  • Community Health: BSN.
  • GI Lab: BSN.
  • Hoag Medical Group: BSN.
  • Women’s COE: BSN.


Required Certifications & Licensure:

  • Registered Nurse in the State of California - Current and in good standing.
