Now Hiring - Clinical Liaison - East Longmeadow, MA
$50,000 - $80,000/annually
Compensation will be based on, but not limited to, experience, qualifications, credentials and any other relevant information
CareOne at Redstone
The Clinical Liaison will be responsible for facilitating patient admissions via marketing and education activities with contracted and potentially contracted hospitals, skilled nursing facilities, respite centers, physician, and other community resources. This highly visible role will act as the point of contact for health provider discharges and facilitate the admissions process for all patients by working collaboratively, efficiently, and in a professional manner with all levels of staff and community partners. Some of the specific job duties include, but are not limited to:
  1. Visiting hospitals and other health care providers on a daily basis
  2. Creating working relationships with health care and community partners
  3. Serve as a liaison with health care professionals (i.e. physicians, discharge planners, social workers, health care coordinators, etc.) and agency employees to coordinate safe transitions from medical facilities to the home setting/facility setting.
  4. Screening patient clinical charts for appropriate placement, verify payer source and discharge plan
  5. Medication review and follow up to assure patient's smooth transition
  6. Marketing/social media
  7. Provides Sub-Acute in-service programs to hospital and skilled nursing facility staff, physicians, community groups and others ongoing or as directed
  8. Works with team to develop, establish, and maintain clinical services to contracted and potentially contracted hospitals, nursing facilities, area physicians, other agencies, and alternative care programs with potential linkage for LTC/SNF - Sub Acute care
  9. Conducts all pre-discharge meetings, interviews, and meet and greets within designated accounts and assigned territories; may assist other locations as well
  10. Attend care plan meetings with health care provider personnel on behalf of the agency to assure that the patients hospice needs are clearly identified and understood prior to discharge
  11. Inform health care providers facilities personnel of case acceptance and document patient intake information (to include demographic, clinical, and payer information.)
  12. Provide management with weekly and/or monthly updates on referrals and statuses utilizing various methods of reporting and analyzing data; making changes in plans as necessary.
  13. HCRM compliance and utilization daily/weekly.
  14. Assists with the development of annual goals and objectives
  15. Other duties as assigned
  • Bachelor's Degree in Marketing, Business, Communications or related field
  • 3-5 years' experience in sales and marketing and/or related healthcare background
  • Previous experience working in a health care environment required
  • Sales and Marketing experience strongly preferred.
  • Sub-Acute and Skilled Nursing Facility experience strongly preferred
  • Must exhibit high degree of patience and understanding dealing with residents, patients, families, and community partners in handling stressful health/care related concerns, exhibiting strong customer service skills and very strong analytical skills
  • Must successfully demonstrate strong interpersonal, collaborative and organizational skills including effective communications and presentation skills, exhibiting willingness to meet overall performance goals, modeling a high level of professionalism at all times
  • Must exhibit strong orientation to detail with a high degree of self-motivation often multi-tasking, using multiple sources of data and communications tools to accomplish best outcomes for patients and overall performance of the organization
  • Must have knowledge of Medicare/Medicaid regulations related to care
  • Must have a valid driver's license and reliable automobile
  • Must be comfortable in environments where there is potential exposure to infection, odors and communicable diseases.
