Job description

Nurse Practitioner Or Physician Assistant

Community Health is seeking a Nurse Practitioner or Physician Assistant to work at our Rutland, Castleton and Brandon clinics. Candidates are expected to be knowledgeable and skilled in the delivery of primary care. Responsibilities include, but are not limited to:

  • Obtains complete medical history and physical examination on patients.
  • Reviews medical records.
  • Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures.
  • Establishes presumptive diagnosis.
  • Orders appropriate laboratory and diagnostic procedures.
  • Develops a therapeutic plan for patient.
  • Sutures wounds.
  • Prepares written prescription orders for drugs and controlled substances.
  • Instructs patients and family regarding medications and treatment instructions.
  • Provides recommendations regarding health and illness prevention.
  • Recommend community resource to meet patient/family needs.
  • Performs independent evaluations and treatment procedures in emergency situations.
  • Provides monitoring and continuity of care between physician visits.

Knowledge, Education and Skill Requirements:

  • 3-5 years Primary Care Practice Experience.
  • Graduate of an accredited Acute Care Nurse Practitioner or Physician Assistant Program.
  • Licensed for Nurse Practitioner or Physician Assistant license in the state of Vermont.
  • Strong inpatient skill set.
  • Quality-focused team player.
  • Excellent organizational, time management and problem-solving skills.
  • Ability to function effectively as part of a multi-disciplinary team.
  • Effective oral and written communication skills.
  • Commitment to providing exceptional patient care.
