What is an abstractor?

Abstractors are professionals who review and summarize public records, legal documents, or other data, often focusing on property titles or legal proceedings. Their main role is to identify and compile key information from complex documents into concise reports. Abstractors are commonly employed in real estate, law, or medical fields, ensuring that information such as ownership history, liens, or legal claims is accurately documented. This work is essential for verifying property ownership, conducting legal due diligence, or maintaining accurate records in various industries.

What are the key skills and qualifications needed to thrive as an abstractor, and why are they important?

To thrive as an Abstractor, you need a keen eye for detail, strong analytical skills, and a solid understanding of legal documents or medical records, typically supported by relevant experience or training. Familiarity with specialized software for document management, databases, and sometimes certification in medical coding or legal research is often required. Excellent organizational skills, critical thinking, and effective communication set top performers apart in this field. These skills ensure accurate data extraction, reduce errors, and support smooth workflow in legal, real estate, or healthcare environments.

What are some common challenges abstractors face when working with large volumes of complex records?

Abstractors often encounter challenges such as deciphering handwritten or poorly scanned documents, managing inconsistencies in record formats, and ensuring accuracy while working under tight deadlines. Large volumes of complex records can require strong attention to detail and effective organizational skills to avoid errors. Collaboration with other team members, such as attorneys or title examiners, is often necessary to clarify ambiguous information and ensure all relevant data is properly extracted and documented.

What are abstractor jobs?

An abstractor works in the real estate industry or the healthcare industry. A clinical data abstractor organizes and analyzes data using medical charts and other patient information. They may do this for clinical purposes or to support medical researchers. A title abstractor, also known as a land abstractor, uses databases to make sure that the title for real estate is valid, and that no other liens or claims are on the property. The duties of someone in this job require the ability to perform research and organize data.

What is the difference between Abstractor vs Title Examiner?

This full-time role is ideal for candidates who have already served as an abstraction lead, registry lead, quality lead, or similar role, as well as experienced healthcare abstractors ready to move beyond primary chart abstraction into a client-facing account coordination and oversight role.

The Account Lead serves as the primary point of contact for assigned client accounts and ensures accurate, timely, and well-coordinated abstraction work across all assigned measures, registries, abstractors, client systems, and deadlines. This role supports account health, client satisfaction, abstractor coordination, documentation standards, quality review, billing readiness, and issue escalation. This position does not include direct reports.

This is not a general data entry or abstractor position. Candidates must have direct healthcare registry, clinical chart review, quality reporting, or data abstraction experience. The role requires strong organization, communication, prioritization, follow-through, problem-solving, and client relationship skills.

Position Details: This is a full-time position. ADN is hiring for remote or onsite candidates. Remote candidates must be available during normal business hours, Monday through Friday, to support client communication, internal operations, and account needs.

Key Responsibilities:

  • Serves as the primary contact for assigned data abstraction client accounts, including routine communication, deadline coordination, client questions, account updates, and feedback.
  • Coordinates account activity across assigned abstractors, client contacts, DAS Management, and internal teams to help ensure accurate, timely, and complete abstraction work.
  • Monitors account progress, deadlines, documentation, and completion status; follows up with abstractors and escalates barriers or concerns early when needed.
  • Maintains accurate and timely client account documentation, including trackers, preferences, and status updates to support progress monitoring, billing readiness, quality review, and client communication.
  • Supports onboarding of new abstractors to assigned client accounts, including client-specific workflows, preferences, and expectations.
  • Conducts and/or coordinates inter-rater reliability reviews, validation activities, feedback, correction, and follow-up to support abstraction accuracy and consistency.
  • Performs abstraction, IRR, gap coverage, or other chart review duties as needed based on client needs, staffing, deadlines, or account priorities.
  • Communicates professionally and proactively with clients, assigned abstractors, DAS Management, and internal teams.
  • Builds strong working relationships with clients by responding timely, maintaining routine touchpoints, identifying trends or opportunities for improvement, and helping clients view ADN as an extension of their quality team.
  • Maintains strict confidentiality and complies with ADN, client, HIPAA, and data security requirements.

Qualifications:

Required:

  • At least 2 years of recent healthcare quality abstraction, clinical registry, chart review, or data abstraction experience.
  • Experience with one or more clinical quality measures, registries, or abstraction programs, such as Core Measures, NCDR, STS, GWTG, NSQIP, NHSN, trauma, or other related registries.
  • Strong understanding of healthcare quality reporting, registry abstraction, clinical documentation review, medical record interpretation, and abstraction accuracy expectations.
  • Excellent written and verbal communication skills, including the ability to communicate professionally with clients and internal team members.
  • Strong organization, time management, documentation, follow-through, and attention to detail.
  • Ability to coordinate multiple priorities, deadlines, systems, and workflows while remaining responsive and accountable.
  • Ability to identify barriers, solve problems, escalate concerns appropriately, and work collaboratively with DAS Management.
  • Comfort working in multiple EHRs, registry platforms, vendor tools, spreadsheets, trackers, Google Drive, JIRA, Confluence, and other data collection or project management systems as assigned.
  • Availability during normal business hours, Monday through Friday, even if working remotely.

Preferred:

  • Prior experience as an abstraction lead, registry lead, quality lead, account lead, team lead, or similar role in a hospital, health system, vendor, or healthcare quality organization.
  • Experience supporting or coordinating abstractors, reviewing abstraction quality, conducting IRR, mentoring team members, or managing abstraction timelines.
  • Experience with client communication, account coordination, documentation tendency review, workflow development, audit support, or abstraction validation.
  • Experience with multiple measure sets, registries, EHRs, vendor tools, or healthcare quality programs.
  • Bachelor's degree, LPN, RN, RHIT, RHIA, CPHQ, or other healthcare quality, clinical, or registry-related credential.

Screening & Onboarding: Qualified candidates will complete an initial phone screen and measure/registry-specific abstraction assessments based on the experience listed in their application. These assessments are designed to evaluate current knowledge of specifications, abstraction judgment, accuracy, and completion speed.

ADN does not provide basic abstraction training, so candidates should already have the measure/registry experience needed for the role. After hire, ADN provides onboarding to ADN operations, account lead expectations, client systems, EHR access, documentation tendencies, and other client-specific workflows relevant to assigned accounts.

Why This Role May Be a Good Fit: This position may be a strong fit for experienced abstractors, registry leads, or quality professionals who enjoy account ownership, client communication, abstractor support, quality oversight, and operational coordination. This role is ideal for someone who is organized, responsive, detail-oriented, and comfortable balancing client expectations, abstractor needs, deadlines, documentation, and quality requirements.

ADN offers a knowledgeable and supportive team environment, meaningful work that supports hospitals and patient care, and the opportunity to contribute to a growing healthcare quality service line.
