• Conduct thorough evaluations of patients, including reviewing medical histories and performing specific assessments to formulate effective treatment plans and document results comprehensively.
• Communicate evaluation findings and treatment plans clearly to both patients and family members, engaging them in the goal-setting process to foster collaborative care.
• Implement targeted treatment protocols based on evaluation findings, adapting techniques to align with the latest healthcare standards and the individual needs of patients.
• Collaborate with other healthcare professionals to address patient needs, documenting any changes in the patient's condition and adjusting the plan of care accordingly.
• Manage discharge planning and follow-up care by maintaining accurate and timely documentation that reflects the patient's full course of care, including initial evaluations, progress notes, and discharge summaries.
• Ensure effective time management in delivering care to meet productivity targets while maintaining a consistent and organized schedule.
• Actively participate in team meetings and community marketing initiatives to enhance service visibility and outreach.
• Contribute to setting and achieving departmental goals while complying with relevant regulations and standards.
• Engage in ongoing professional development, pursuing continuing education opportunities to enhance personal competencies and those of the organization.
• Promote a positive patient experience by demonstrating flexibility and adaptability in response to patient feedback and needs.

You will serve a diverse patient population encompassing individuals recovering from orthopedic surgeries, athletic injuries, and chronic pain conditions. Our clinical approach prioritizes individualized care and progressive rehabilitation strategies, ensuring each patient's unique needs are met with compassion and expertise.

• A Doctorate degree in Physical Therapy from an accredited program is required.
• A High School Diploma or GED from an accredited institution is necessary.
• Current state licensure for physical therapy is essential for practice.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association is mandated.
• Although prior work experience is not required, a commitment to professional growth and skill development is expected.

• Experience in orthopedic rehabilitation or home health settings is a plus, but we welcome candidates eager to advance their careers in orthopedic physical therapy.
• Familiarity with electronic health record (EHR) systems for documenting patient information and treatment plans.

This position offers a competitive salary range of $100,000 to $120,000 annually, depending on experience and qualifications.
