Community Case Manager, RN

Candidate must be a clinically competent registered nurse. The CCM nurse works with patients to educate them on chronic disease management in the patient’s home. This position will also have a special focus on diabetes education. Candidate counsels the patient and family in meeting nursing and related needs, participates in in-service programs, supervises and teaches other nursing personnel. Graduate of an accredited school of nursing currently licensed in the State of Ohio.

Responsibilities

  • Provides direct patient care
  • Provides education to patients/family on chronic disease processes
  • Provides direct consultation using in-depth clinical knowledge and skills in area of Diabetes to groups and individual patients with Type 1, Type 2, Gestational Diabetes and Pre Diabetes patient
  • Assists in Diabetes Self-Management Education and Support (DSMES) program as well as Diabetes Prevention Program (DPP)
  • Receives and assesses referrals from referring providers.
  • Consults with multidisciplinary team to develop, implement and evaluate care for patients and families.
  • Documents patient care activities in patient medical records
  • Complete documentation accurately and timely.
  • Coordinate patient care with appropriate disciplines.
  • Physician consultation for changes in patients' condition.

Requirements

  • Registered nurse with two years of experience in delivering nursing care
  • Current CDCES/CDE or willingness to obtain certification
  • Analytical abilities and sound nursing knowledge.
  • Effective communication skills.
  • Knowledge of policies and procedures.
  • CPR certification.
  • Reliable transportation, current driver's license, and automobile insurance.
  • LMH is accredited by DNV and TJC, and as such, may require specific annual education related to specialty certifications and standards.
