Job description

Program:  Marine Terrace | Location:  Astoria, New York

Position Summary:

The Veteran Housing Navigator provides comprehensive, on-site supportive services to residents of Marine Terrace, a 53-unit residential building dedicated to low-income Veterans.  This position is responsible for promoting housing stability, independent living, and overall wellness through coordinated case management, service referrals, and community-building initiatives.

Reporting to the SSVF Assistant Program Director, the Veteran Housing Navigator works closely with internal programs including Home4Good and Supportive Services for Veteran Families (SSVF) to ensure integrated, wraparound service delivery. The Navigator assists residents in accessing benefits, healthcare, employment services, and community resources, while tracking outcomes and maintaining compliance with program and funder requirements.

Duties and Responsibilities:

Direct Service & Case Management

  • Provide on-site and wraparound supportive services to Veteran tenants.
  • Conduct intake assessments and gather required documentation for program databases.
  • Assist Veterans in transitioning from military service to independent community living.
  • Identify resident needs and connect tenants to appropriate services and community resources.
  • Develop individualized Housing Stability Plans for residents requiring intensive support, including goal setting and progress monitoring.
  • Review and update Housing Stability Plans monthly and adjust service strategies as needed.

Service Coordination & Referrals

  • Coordinate social service programming and community events for residents.
  • Provide referrals to the Veterans Administration (VA) for health care, mental health services, substance use treatment, physical therapy, and VA benefits.
  • Refer residents to career counseling, job placement services, educational and vocational training programs, benefits counseling, financial literacy workshops, and legal services.
  • Facilitate or coordinate support groups and counseling services as appropriate.

Health & Wellness Promotion

  • In coordination with the SSVF Healthcare Navigator, partner with local and city agencies to promote healthy lifestyles among Veteran tenants.
  • Coordinate health screenings, workshops, and referrals related to physical health, mental health, substance use recovery, and nutrition.
  • Connect residents to affordable healthy food resources and other wellness initiatives.

Community Engagement

  • Support Veteran-specific events and activities such as Veterans Day, Memorial Day, and other community recognition programs.
  • Foster a respectful, inclusive, and supportive community environment.

Collaboration & Administration

  • Maintain effective communication between tenants and property management in coordination with Home4Good and SSVF.
  • Accurately document services, case notes, and outcomes in required databases.
  • Track program impact and outcomes in compliance with agency and funder requirements.
  • Establish and maintain partnerships with community-based organizations to enhance service delivery.

 

Qualifications:

Education & Experience

  • Bachelor’s degree in Social Work, Human Services, or a related field preferred.
  • Minimum of two (2) years of experience working with Veterans, low-income individuals, or people experiencing housing instability.
  • Knowledge of VA systems, Veteran benefits, and community resources strongly preferred.
  • Experience in case management, service planning, and documentation required.

Skills & Competencies

  • Strong interpersonal, communication, and organizational skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Proficiency in database entry and documentation systems.
  • Knowledge of trauma-informed care principles.
  • Commitment to serving diverse populations with dignity and respect.
  • Spanish proficiency desirable but not required.
