Job description

Social Work p.r.n. is recruiting for a full-time, direct hire position located at a local long-term care facility in Valley Park, MO.

The Social Worker manages the facility?s social service program. This role focuses on helping residents navigate and solve their social, emotional, and economic challenges through counseling, advocacy, and comprehensive care planning.

Essential Duties:

  • Resident Support & Advocacy: Assists residents with moving in, adjusting to the facility, and actively advocating for their rights. Evaluates psychological needs and helps residents and families understand and accept illnesses or disabilities.

  • Documentation & Compliance: Manages admissions paperwork, assessments, care plans, annual and quarterly summaries, and behavioral charting. Ensures strict adherence to state/federal regulations, corporate compliance policies, and ethical standards.

  • Collaboration & Continuity of Care: Coordinates functions across departments and maintains close relationships with the interdisciplinary team including physicians, nursing staff, and community health/welfare agencies. Participates in discharge planning and handles community resource referrals.

  • Grievance & Incident Resolution: Promptly investigates resident grievances, tracks incident reports, and provides oral and written findings in compliance with state guidelines.

  • Administrative Duties: Performs daily clerical tasks, coordinates resident appointments, assists with field trips, and orders supplies.
