Service Coordinator, Philadelphia County

Service Coordinator needed for full-time, temp to hire position in Philadelphia County. Responsibilities include conducting person-centered assessments, developing individualized service plans, coordinating resources, monitoring services, and evaluating outcomes for individuals with disabilities and older adults. You'll also facilitate care transitions, coordinate with behavioral health services, assist with complaints and grievances, maintain CHC eligibility for participants, complete critical incidents, adhere to HIPAA regulations, utilize tablets for fieldwork and reporting, and participate in quality improvement activities, professional development, and team meetings.

BSW with a minimum of three years' experience in social services or healthcare. Valid driver's license, good driving record, and continuous access to a fully insured car.
