Job Summary

The RN-Patient Flow Coordinator operates within Prisma Health Patient Flow Operations, encompassing Central Bed Management and the Transfer Center. Central Bed Management oversees the placement of inpatient and observation patients by reviewing admission/discharge/transfer (ADT) orders and bed requests in coordination with clinician assessments and triage. The Transfer Center facilitates direct admissions and patient transfers from both Prisma Health and external facilities and physician practices. Together, the Prisma Health Patient Flow Operations teams coordinate patient admissions, transfers, and placements in alignment with each facility or unit's capacity and capabilities.

Essential Functions

  • All team members are expected to be knowledgeable and compliant with Prisma Health's purpose: Inspire health. Serve with compassion. Be the difference.

  • Applies standardized processes, nursing autonomy, remote triage and patient acuity to ensure patients are admitted to the appropriate unit/facility with correct orders.

  • Utilizes critical thinking skills to prioritize patient needs, in conjunction with standardized processes for patient admissions, transfers, and discharges.

  • Monitors patient throughput turnaround times to facilitate timely movement and maximizes use of existing systems and processes to improve patient flow.

  • Utilizes automated and manual information to forecast and facilitate bed availability.

  • Supports initiatives to enhance patient throughput and promote safe, timely care transitions.

  • Possesses excellent communication skills, ensuring oral communication is clear and concise.

  • Acts as a resource for other staff members by maintaining a strong clinical knowledge and understanding of individual department capabilities.

  • Delegates tasks as appropriate to other members of the team and ensures completion of assigned tasks.

  • Collaborates with physicians, clinical providers, emergency departments, inpatient units, Administrators on Duty, environmental services, and other members of the interdisciplinary care team to improve care transitions.

  • Evaluates own nursing practice in relation to professional practice standard guidelines, state statutes, policies, procedures, rules/regulations and current specific knowledge. Takes personal accountability for maintaining competency through education and professional activities. Maintains professional licensure and completes required mandatory education in a timely manner. Reflects on identified areas of strength that offer opportunity for improvement.

  • Performs other duties as assigned.

Supervisory/Management Responsibilities

  • This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

  • Education - Associate degree in Nursing. Bachelor's degree in nursing preferred.

  • Experience - Two (2) years Acute Care Nursing experience. ICU or ED nursing experience preferred.
