Enhance your Family Care career in this Manager, Care Coaching role. Reporting to the Associate Director, Care Coaching, you will support the work of Care Management staff. You will provide daily leadership and mentoring to staff. This position supports Jefferson County, WI and surrounding areas/counties.
Main Responsibilities:
- Mentor and support the work of Field Nurses and Care Coaches (or IDT) to ensure the Family Care benefit is delivered in compliance with the DHS contract.
- Identify learning needs; arrange for mentorship, provide clear expectations, facilitate orientation and ongoing learning to build capacity of IDT staff.
- Assess and coach staff on the IDT primary competencies and performance standards.
- Review reports, assess process and review documentation.
- Assess utilization and the quality of care management for each member served.
- Participate in cross departmental collaboration to ensure the quality and effectiveness of provider services.
- Educate community partners.
- Participate in the hiring process by selecting, orienting, and training associates.
- Facilitate in person unit meetings and attend quality visits as needed.

Required Qualifications
- Bachelor's degree in human services or a related field or associate degree in nursing.
- 1 year of leadership experience in the human services field.
- 3 years of experience working in the Family Care program.
Preferred Qualifications
- 5 years of experience working with individuals with intellectual/developmental disabilities, physical disabilities or frail elders.
- Reside within 2 hours of Jefferson, WI
Additional Information
- Workstyle: Work remote from home
- Travel: at least 1 time per month with potential for more
- Typical Work Days/Hours: Monday - Friday, 8:00 am - 4:30 pm CST
