Job description

Activation Worker - Temporary part-time for 4 months, with possibility of extension

Under the supervision of the Activation Manager, the Activation Worker plans and conducts daily activation, recreational and social programs for residents of the LTC Home that reflects the organization's resident centred approach to improve and maintain the residents' quality of life in the Home.

Qualifications:

  • Diploma in Recreation and leisure studies, therapeutic recreation, kinesiology, or equivalent

  • Experience in a social services or long-term care setting an asset

  • Experience and skills in facilitating activation and therapeutic recreation programs, preferably working with the geriatric population

  • Creativity and innovativeness in developing meaningful programs for clients within a tight budget

  • Superb customer service skills. Demonstrated initiative to support the clients and team in a positive and efficient manner.

  • Computer literacy. Knowledge of Chinese word processing, an asset

  • Current CPR Certificate

  • Current Food Handler Safety Certificate

  • Satisfactory Police Vulnerable Sector Screen

  • Ability to communicate in English and Chinese

Responsibilities:

  • Conducts timely and accurate assessment of residents upon admission and on a quarterly basis. Prepares and monitors effectiveness of care plan

  • Plans and implements innovative regular and seasonal recreational and social activities to improve residents' quality of life, taking into consideration their feedback, needs and preference, and ensures activities are conducted in a safe, hygienic and supportive environment, including one-on-ones, small group and large group activities

  • Assists the Activation Manager in supervising volunteers who assist or facilitate activation programming

  • Mentors and works effectively with students and interdisciplinary team members

  • Maintains and updates resident records

  • Works with staff of other disciplines in the LTC Home in providing quality care to LTC Home residents

  • Shops for cost effective and appropriate program supplies and maintains accurate record of fees and expenditure

  • Assists the Activation Manager in providing information and education to residents and their family members (hand hygiene workshop)

  • Participates in in-service programs and professional development

  • Keeps up-to-date with developments and best practices in the field

  • Other duties as assigned
