Position Overview

Title: Spiritual Care Coordinator (Chaplain)
Reports To: Clinical Director or Designee
FLSA Status: Exempt
Work Location: On the Road

The Spiritual Care Coordinator provides compassionate, patient-centered spiritual care to hospice patients, families, and caregivers. This role collaborates closely with the Interdisciplinary Team (IDT) to ensure holistic, culturally sensitive care aligned with hospice philosophy and regulatory standards.

Key Responsibilities
  • Assess spiritual needs and provide supportive spiritual care to patients and families
  • Develop, implement, and document individualized spiritual care plans
  • Collaborate with the Interdisciplinary Team to coordinate holistic care
  • Participate in IDT meetings and care plan reviews
  • Provide consultation, education, and support to staff regarding spiritual care
  • Coordinate with clergy, faith leaders, and community spiritual resources as requested
  • Conduct pre-bereavement and bereavement assessments, visits, and follow-ups
  • Facilitate or conduct funeral and memorial services for patients, families, facilities, and staff as requested
  • Network with faith communities to educate on hospice philosophy
  • Supervise spiritual care volunteers as assigned
  • Manage caseload, scheduling, and travel efficiently
  • Respond flexibly to unscheduled events, admissions, or emergencies
  • Maintain compliance with all local, state, federal, and Medicare regulations
  • Participate in evening, weekend, holiday, and on-call rotations as required
  • Maintain professional, ethical conduct and regular attendance
  • Foster a positive, respectful, and engaging work environment
Qualifications
  • Minimum age of 18
  • Graduate of an accredited seminary or school of theology (B.D., M.Div., or equivalent), or degree in counseling
  • Experience working with death and dying
  • Ability to work effectively within an interdisciplinary team
  • Comfort working in an interfaith, multicultural environment
  • Knowledge of and commitment to hospice philosophy
  • Hospice or home health experience preferred
  • Strong communication, planning, and organizational skills
  • Self-directed with the ability to work independently
  • Valid driver's license, reliable insured transportation, and acceptable driving record
  • Ability to meet physical and health requirements, including TB screening and physical exam per state and company policy
  • Satisfactory professional references
