NURSE PRACTITIONER (NP) - IN-HOME HEALTH RISK ASSESSMENTS

Position: PRN / 1099 Contractor
Exclusions: PMHNP, Women's Health, Pediatric NP's and PA's.
Schedule: Flexible (50+ hours/month)
Compensation: $100-$115 per visit (Tiered based on availability)
Territory:Cuyahoga and Lorrain Counties

We are seeking Nurse Practitioners (Certain NPs only; no PAs) with no less than 40 hours per month who can start immediately completing in-home Health Risk Assessments (HRAs) for Medicare Advantage members. Must be able to complete credentialing paperwork, training, and onboarding within the first 10-12 days and start field-based work immediately after thereafter.

This is a pure assessment role: No prescribing, no on-call, and zero follow-up care.

THE PAY BREAKDOWN
  • 60 hrs/mo: $100/ visit
  • 70 hrs/mo: $105/ visit
  • 80 hrs/mo: $110/ visit
  • 90 hrs/mo: $115/ visit
