This position is primarily remote, allowing you the flexibility to work from home while also providing outpatient services to clients. Key Responsibilities

  • Interpret and effectively communicate essential patient and family information to staff and other health providers, ensuring that social history data is accurately conveyed.
  • Coordinate care planning, offer insightful recommendations, and facilitate decision-making for individual patient care plans, with a strong emphasis on the home environment and family dynamics.
  • Develop comprehensive treatment plans, regularly review and monitor client progress, and work collaboratively with providers to enhance therapeutic approaches.
  • Maintain meticulous progress notes and documentation post-session, ensuring compliance with regulatory standards and contributing to high-quality patient care.
  • Participate in multidisciplinary team meetings to discuss case management strategies and share insights on best practices for client treatment.

Patient Population / Client Focus
