Prime Care Wellness Institute is actively seeking a dedicated and compassionate Physical Therapist to contribute to our rehabilitation team at our state-of-the-art facility in Keizer, Oregon. In this full-time position, you will be responsible for developing personalized rehabilitation strategies that enhance the physical capabilities of our clients, particularly senior residents, thereby improving their overall quality of life.

As a Physical Therapist at our Rehabilitation Center, you will work in a dynamic, on-site environment where collaboration with a team of skilled professionals is essential. Our facility is equipped with advanced rehabilitation tools and technologies, creating a safe and engaging atmosphere for both staff and residents. You will typically work Monday through Friday from 9 AM to 5 PM, allowing you to maintain a healthy work-life balance while offering comprehensive care.

• Conduct thorough assessments of residents' physical conditions, including mobility, strength, and balance, to identify their specific needs.
• Develop and implement individualized treatment plans that are tailored to support each resident’s rehabilitation goals.
• Employ a variety of therapeutic techniques, including manual therapy, strength training, and use of adaptive equipment, to help residents regain function.
• Provide pain management strategies and therapeutic interventions aimed at minimizing discomfort and enhancing mobility.
• Educate residents and their families on safe mobility practices, home exercise programs, and rehabilitation techniques to ensure continuity of care.
• Maintain accurate and timely documentation of treatment progress and outcomes in compliance with regulatory standards.
• Collaborate closely with nursing staff and other healthcare professionals to deliver cohesive and coordinated care across departments.
• Promote preventive care through actionable fall prevention strategies and interventions designed to enhance overall resident safety and independence.

This role primarily focuses on the geriatric population residing in our assisted living facility. Many residents face challenges stemming from age-related ailments such as arthritis, post-surgical rehabilitation needs, neurological disorders, and balance issues. By fostering a supportive rehabilitation journey, you will help empower these individuals to achieve their personal health goals and maintain their independence for as long as possible.

• A Doctorate (DPT) or Master’s degree in Physical Therapy from an accredited institution.
• An active Physical Therapy license in Oregon, with eligibility for treatment in geriatric populations.
• Strong foundational knowledge of mobility rehabilitation, pain management modalities, and fall prevention strategies.
• Excellent verbal and written communication skills to effectively engage with residents, families, and healthcare team members.
• A collaborative mindset and ability to work within an interdisciplinary team to optimize resident outcomes.
• Proficiency in electronic health record (EHR) management and clinical documentation practices.

• A minimum of 2 years of experience in a physical therapy role with a focus on geriatric or assisted living settings.
• Demonstrated experience in managing complex rehabilitation cases and delivering tailored treatment interventions.
• Familiarity with clinical best practices and relevant physical therapy methodologies.
