At Astoria Senior Living, an Agemark Senior Living community, you'll have the opportunity to lead a dedicated team while making a meaningful impact on the lives of seniors every day.

For more than 35 years, Agemark Senior Living has been committed to exceptional resident care and creating communities where people thrive. We're proud to be recognized as a Certified Great Place to Work and one of the nation's leading senior living providers.

We're looking for a compassionate, experienced LPN or RN to serve as our Health & Wellness Director—someone who leads with integrity, builds strong relationships, and is passionate about providing exceptional care.

Why You'll Love This Role

Lead with Purpose – Guide and mentor a dedicated wellness team while setting the standard for compassionate, resident-centered care.

Make a Lasting Impact – Build meaningful relationships with residents and families, ensuring every individual receives the care and dignity they deserve.

Grow as a Leader – Partner with community leadership while continuing to develop your clinical and leadership skills.

Create a Culture of Excellence – Foster teamwork, accountability, and a positive environment where residents and team members thrive.

What You'll Do
  • Lead, mentor, and support the wellness team while promoting a culture of excellence.
  • Oversee resident assessments, admissions, service plans, and ongoing care coordination.
  • Ensure residents receive exceptional, person-centered care that promotes independence and quality of life.
  • Communicate effectively with residents, families, physicians, pharmacies, and healthcare providers.
  • Supervise medication management and ensure safe medication administration.
  • Maintain compliance with state regulations, licensing requirements, and company policies.
  • Monitor documentation, resident records, and care plans for accuracy and compliance.
  • Respond to resident concerns, emergencies, and changes in condition.
  • Identify potential risks and implement proactive care and safety plans.
  • Participate in Manager on Duty rotations and provide leadership support as needed.
What We're Looking For
  • Active Nebraska LPN or RN license required.
  • Minimum of 1 year of experience working with seniors or in long-term care, assisted living, or a related healthcare setting.
  • 2+ years of leadership or management experience preferred.
  • Strong knowledge of medications, treatments, and senior health needs.
  • Excellent communication, organizational, and problem-solving skills.
  • Comfortable using electronic documentation and computer systems.
  • Compassionate, dependable, and committed to providing exceptional resident care.
Why You'll Love Working at Agemark
  • Comprehensive Benefits – Medical, dental, vision, employer-paid life insurance, prescription discounts, telehealth, and nurse line access.
  • Work-Life Balance – PTO starts accruing on day one, free meals during your shift, and an Employee Assistance Program offering emotional, financial, and legal support.
  • Financial Flexibility – Daily Pay gives you access to your earned wages when you need them, plus discounted pet insurance and additional voluntary benefits.
  • Professional Growth – Leadership development, continuing education, certification and licensure support, tuition reimbursement, Executive Director in Training opportunities, and up to 100% free college tuition through partner schools.
  • Recognition & Rewards – Employee referral bonuses, recognition programs, and exclusive discounts on travel, entertainment, electronics, and more.
Join a Team That Cares
