Wellmore Health Center is seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our dynamic team in Phoenix, Arizona. Key Responsibilities
  • Act as a liaison between families and outpatient providers, facilitating communication and ensuring that all parties are aligned in support of the student's mental health journey.
  • Engage with insurance companies to manage pre-certification processes and navigate utilization management, ensuring that clients receive appropriate care and resources.
  • Participate in discharge planning and aftercare strategies to ensure seamless transitions for clients moving in and out of services.
  • Offer therapeutic meal support, helping clients develop a healthy relationship with food through modeling and guided practice.
  • Provide empathetic, affirming care tailored to each client’s situation, taking into account their unique identities and experiences.
Patient Population / Client Focus

The individuals you will work with range from elementary-aged students to older adolescents who may face various mental health challenges including anxiety, depression, and behavioral issues. Qualifications
